Moving toward Personalisation:
Many people who use health and social care services and their family carers are asking for more personal control over the services they receive.
The government is pushing for more personalisation, and in Central Lancashire, more organisations and services are striving to meet the challenges of personalisation.
The government says that personalisation means "every person who receives support, whether provided by statutory services or funded by themselves, will be empowered to shape their own lives and the services they receive in all care settings"
Person Centred Thinking and Planning are vital if personalisation is going to succeed in enabling people to lead full productive and interesting lives as valued citizens.

Personalisation always starts with good listening - person centred thinking tools can help with this.
Good listening is helped by good questions, that try to find out who you are, what your gifts and skills are, how you want to live, and where you want to go in life. Then it's about planning what it will take to make this happen.
Personalisation can mean lots of different things:

Personalisation could mean choosing your own staff - the 'matching tool' can help with this

Personalisation can be about how you manage your health and wellbeing. A 'Health Action Plan', and soon perhaps a 'Personal Health Budget' can help with this.
 Personalisation can mean managing your own support budget, so that you are in charge of how you are supported and what for. Everything you and your allies have found out by using person centred thinking can be put together into a 'support plan' that shows how you will use your money to live the kind of life that makes sense for you.
